Where to start ?
We invite you to contact someone at the Salon Funéraire Roland Ménard Inc (at all times).
A member of our team will coordinate the recovery of the remains. We will then fix a date to meet and plan the funeral procedures.
The encounter
Here is what we will ask of you: Health insurance card, social insurance card, place of birth, date of union (if applicable), names of the individuals left mourning.
(You will find the Reminders document, to be completed and brought in person during our first meeting, HERE.)
Objects to bring during your meeting: Photo, clothes, jewelry, and other objects you wish to place with the deceased (optional).
Various subject matters will be approached during our meeting: Funeral ceremony type, location of the ceremony, dates, duration, burial type, celebrant type, floral arrangements, reception room, caterer, location of the interment, choice of bookmarks, videos and/or photos montage, respects, music, etc.
Here is the list of documents that will be filled for you:
1. Old Age Security Pension
2. Form from the Director of Civil Status to obtain a death certificate
3. Form from the Québec Pension Plan (claiming the surviving spouse’s pension)
4. Form from the Registrar of Civil Status with the declaration of death; cancellation of the health insurance card
5. Evidence of death for the settlement of the estate
Here is the information that will be asked:
- Health insurance card,
- Social Insurance Card
- Place of birth, date of marriage (if applicable)
- Names of people left behind.
(You will find an aide-memoire document to fill in and bring during the first HERE meeting.)
Here is the list of documents that will be completed for you:
- Old Age Security Pension
- Form of the Registrar of Civil Status to obtain a copy of the death certificate
- Form of the Québec Pension Plan (application for the pension of the surviving spouse)
- Form of declaration of death with the Directeur de l'état civil; cancellation of the health insurance card
